Hreflang is easy to break during a routine deployment: a route changes, a translation disappears, or a template emits stale alternates. A regression check should compare the intended locale map with the rendered pages after every relevant release. It should fail on a real mismatch and identify the exact URL pair.

Treat legitimate page removals as changes to the approved map, not silent test suppression.
Worked example
Synthetic release: /en-ae/demo/ points to /ar-ae/demo/, but the Arabic route was changed to /ar-ae/request-demo/. A test reports the old target as 404 and a missing return link from the new URL. The release owner updates the map and annotations, then reruns the pair before deployment. This example supplies the failing row and correction rather than claiming a client test.
Limits to keep in mind
Do not require hreflang for every page. The test should cover only real equivalent variants. It should also allow the site to use HTML, HTTP header or sitemap implementation when that choice is deliberate.
